Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Concord Care of Hartford. As a key member of our healthcare team, you will provide high-quality patient care and support to our residents.

Key Responsibilities:

  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Participate in ongoing education and training to stay current on best practices and industry developments.
  • Contribute to a positive and supportive work environment, promoting a culture of respect, empathy, and professionalism.

Requirements:

  • Valid Ohio State License as a Licensed Practical Nurse.
  • Current certification in Basic Life Support (BLS) and CPR/AED.
  • Previous experience in long-term care or a related field, with a focus on patient-centered care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to lift, push, and pull up to 25 pounds, with frequent standing, walking, and bending.
