Job Description

As a Mental Health Care Navigator, you will be responsible for providing intake and assessment services to clients referred to our program. You will work collaboratively with a team of professionals to develop and implement Individual Service Plans, monitor progress, and provide support through regular contact and home visits.

Required Skills and Qualifications

To be successful in this role, you must possess a Master's degree in a related field or be MHRT/C eligible. A current Maine driver's license and clean driving record are also required.
