Our dynamic team of nurses, aides, social services, and spiritual care professionals work together to provide comfort, solutions, and peace to those in need. As a Hospice Nurse Practitioner, you will provide comprehensive hospice services under the supervision of a collaborating physician.

The ideal candidate will have a Master's degree with a minimum of one (1) year Nurse Practitioner experience and hospice or palliative care experience. They will also have excellent observation, communication, problem-solving, and nursing skills.

Responsibilities include:

• Providing medical care, palliation, and management of terminal illness in compliance with the state Nurse Practice Act
• Conducting history and physical examinations, interpreting test results, diagnosing health conditions, and documenting findings
• Developing individualized plans of care, prescribing medications or treatments, coordinating services, and ensuring symptom control
• Collaborating with the interdisciplinary team to review and revise the patient's plan of care
• Communicating and coordinating with attending physicians, healthcare practitioners, and community resources
• Educating patients, families, and caregivers on preventive care, medical and psychological problems, and spiritual needs
