Are you a highly skilled and compassionate healthcare professional seeking a rewarding opportunity in the hospice industry? We have an immediate opening for a Hospice Nurse Practitioner at Emblem Hospice.
Our dynamic team of nurses, aides, social services, and spiritual care professionals work together to provide comfort, solutions, and peace to those in need. As a Hospice Nurse Practitioner, you will provide comprehensive hospice services under the supervision of a collaborating physician.
The ideal candidate will have a Master's degree with a minimum of one (1) year Nurse Practitioner experience and hospice or palliative care experience. They will also have excellent observation, communication, problem-solving, and nursing skills.
Responsibilities include:
- Providing medical care, palliation, and management of terminal illness in compliance with the state Nurse Practice Act
- Conducting history and physical examinations, interpreting test results, diagnosing health conditions, and documenting findings
- Developing individualized plans of care, prescribing medications or treatments, coordinating services, and ensuring symptom control
- Collaborating with the interdisciplinary team to review and revise the patient's plan of care
- Communicating and coordinating with attending physicians, healthcare practitioners, and community resources
- Educating patients, families, and caregivers on preventive care, medical and psychological problems, and spiritual needs
