Job Summary:

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Signature Healthcare at Home. As a Home Health LPN, you will provide direct patient care under the supervision of a registered nurse, following a comprehensive assessment and the plan of care.

Responsibilities:

  • Provide direct patient care as defined in State Nurse Practice Act.
  • Implement current nursing practice following a comprehensive assessment and the plan of care.
  • Provide accurate and timely documentation of patient services to reflect the plan of care.
  • Evaluate patient's response to treatments/medications.
  • Participate in personal, professional growth and development.
  • Attend required staff meetings and patient care coordination activities.

Requirements:

  • Graduate of an accredited practical nurse or vocational nursing program.
  • One (1) to two (2) years nursing experience preferred.
  • Currently licensed as an LPN/LVN in the State.
