Columbus House, a leading organization in providing services for individuals experiencing homelessness, is seeking a highly motivated Family Triage Case Manager to join our team.

As a member of our team, you will play a critical role in empowering homeless families to end their homelessness. Your expertise and dedication will help us achieve our mission of serving people experiencing homelessness or at imminent risk by providing life-saving outreach, shelter, and housing, and fostering their personal growth and independence.

About the Role

This part-time seasonal position offers an opportunity to work with a dedicated team and make a meaningful impact in the lives of individuals and families in need. As a Family Triage Case Manager, you will provide case management to homeless families, network with local service providers, and collaborate with clinicians and other service providers to ensure seamless support for our clients.

Your Responsibilities

  • Provide linkages for clients who need social, mental health, educational, vocational, recreational, spiritual, and/or substance abuse services.
  • Transport clients to appointments as needed.
  • Coordinate and collaborate with other agencies and social service providers to ensure comprehensive support for our clients.
  • Oversee intake, departures, and daily roster and programs.
  • Attend daily family morning calls (virtual) and participate in weekly and monthly agency meetings.
  • Monitor client cases and complete all required documentation/charting.
  • Enter and maintain accurate client charts in our Homeless Management Information System Database Case Worthy.

About You

We are looking for an individual with a Bachelor's degree in human services and a minimum of two years' experience working in human services or related fields. Excellent organizational and time management skills, strong written and oral communication skills, and proficiency in Microsoft Office and other common desktop applications are essential. Additionally, you should be able to effectively use local service providers, engage with criminal justice systems, manage group dynamics, and possess knowledge of crisis intervention, boundaries, confidentiality, universal precautions, harm reduction, trauma-informed care, and motivational interviewing.

What We Offer
