About the Role

CenterWell Home Health is seeking a skilled Medical Social Worker to join our team. As a Medical Social Worker, you will play a vital role in providing personalized care to patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization.

Key Responsibilities
  • Evaluate and develop a plan of care tailored to each patient's emotional and social needs.
  • Assess patients' social and emotional states, identifying areas where they may require additional support.
  • Collaborate with healthcare teams to ensure comprehensive care and develop effective discharge plans.
  • Supervise and instruct Social Work Assistants to ensure high-quality medical social services.
  • Maintain accurate documentation and records as required by the company and relevant authorities.
Requirements
  • Master's or doctoral degree from a school of social work accredited by the Council on Social Work Education.
  • Social Worker licensure in the state of practice, if required by state law or regulation.
  • A valid driver's license, auto insurance, and reliable transportation.
  • Proof of current CPR certification.
  • Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice.
  • Excellent oral and written communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment and adapt to changing situations.
  • Knowledge of medications and their correct administration.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
