At Res-Care, Inc., our mission is to provide compassionate end-of-life care to patients and their families. We are seeking a skilled RN / Registered Nurse - Hospice On-Call to join our team.
Job Description
The ideal candidate will have 1 year of nursing experience and a commitment to clinical and documentation excellence. As an End-of-Life Care Specialist, you will provide direct care and coordinate with the interdisciplinary team to ensure optimal patient outcomes.
Responsibilities:
- Conduct comprehensive assessments of patients and families to determine nursing diagnoses and develop individualized care plans.
- Administer medication and treatment as prescribed by physicians and counsel patients and families on meeting nursing and related needs.
- Communicate effectively with the healthcare team and participate in interdisciplinary group meetings to address medical and emotional needs.
- Maintain accurate clinical notes and ensure compliance with state and federal regulations.
Qualifications:
- Graduate of an accredited nursing school with current licensure to practice in the state of operation.
- Experience in hospice or a similar setting preferred.
- Knowledge of the hospice philosophy of care and commitment to clinical excellence.
