Job Summary

We are seeking an experienced Registered Nurse (RN) to join our team as a Nurse Care Coordinator. As a key member of our healthcare team, you will play a vital role in supporting patients and healthcare providers to ensure seamless care coordination.

Key Responsibilities:
  • Perform general nursing care to patients by following established clinic workflows.
  • May perform specific nursing care related to the specialty of the practice.
  • Obtain patient consent for care and ensure patient confidentiality.
  • Document health history, chief complaint, vital signs, and health risk factors.
  • Provide patient education on health status, health maintenance, and disease management.
  • Collaborate with healthcare teams to improve patient care processes and outcomes.
Requirements:
  • Current Registered Nurse (RN) license allowing practice in the state.
  • Graduate of an accredited nursing program.
  • Basic Life Support (BLS) certification from the American Heart Association required.
Compensation

The estimated salary for this position is $25-$35 per hour, depending on experience and qualifications.
