Job Summary:

Elderwood is seeking a skilled RN Care Manager to join our team in Buffalo, NY. As a critical resource for our members, you will assess their home health and level of care needs, assist them in accessing necessary covered services, and provide referrals and coordination of other medical services in support of their member-centric service plan.

Responsibilities:

• Partner with the Social Worker to provide comprehensive assessments of members' home health and level of care needs

• Assist members in accessing necessary covered services and provide referrals and coordination of other medical services

• Develop and maintain person-centered service plans based on needs assessments

• Review financial, legal, or medical issues and refer members to social work or other professionals

Requirements:

• Current New York State Registered Nurse License and valid NYS Driver's license

• BSN or AAS Degree or diploma in Nursing and Case Management Certification (preferred)

• Minimum of three years nursing experience in home care, case management, discharge planning, or managed care

• Minimum of one year experience working with a frail or elderly population

• Minimum one year experience with health assessments

What We Offer:

• Competitive salary range of $68,640 - $93,600

• Monday-Friday schedule with no weekends or overnights
