This role focuses on delivering high-quality patient care services in accordance with established medical protocols. As a key member of the healthcare team, you will work closely with registered nurses to provide safe and effective therapy for assigned patients. Your responsibilities will include implementing and evaluating patient care plans, participating in performance improvement activities, and maintaining a collaborative work environment.
Job Overview
As a Licensed Practical Nurse, you will work under the direction of a registered nurse to provide nursing care to patients. You will utilize the nursing process to plan, implement, and evaluate nursing care, collaborating with other healthcare professionals to provide comprehensive care. Your role will involve collecting data on patient health status, identifying health goals, and ensuring care is reflective of the organization's core values.
Job Description
Minimum Qualifications:
1. Massachusetts Licensed Practical Nurse (LPN) license.
2. Experience dependent on unit requirements and needs.
3. Basic Life Support Certification.
Preferred Qualifications:
1. Cardiopulmonary Resuscitation (CPR) Instructor certification.
2. Graduation from an accredited practical nursing program.
Duties and Responsibilities:
1. Implement medical and nursing plans of care, administer medications, and perform procedures under the direction of a registered nurse.
2. Provide direct patient care within the primary nursing model, ensuring continuity of care and maintaining patient rights and dignity.
3. Perform technical aspects of patient care safely, according to established policies and procedures, and the Nurse Practice Act.
4. Assess and reassess pain, utilize appropriate pain management techniques, and document pain management.
5. Communicate changes in patient condition immediately to the registered nurse.
6. Base nursing interventions on pathophysiological and psychosocial rationale, delivering care in a fiscally responsible manner.
7. Establish positive relationships with patients and families, introducing yourself as the patient's nurse, providing teaching and counseling, and documenting teaching on the appropriate teaching tool.
8. Review and educate discharged patients and families on ADLs, medications, treatments, and provide discharge instruction sheets.
9. Document all nursing interventions and patient responses in the medical record for assigned patients.
10. Delegate responsibilities to others, understanding their roles, knowledge, and capabilities.
11. Participate in performance improvement activities, utilizing performance improvement principles to support and improve patient care.
12. Hold yourself accountable for professional practice, presenting yourself in a calm and professional manner, and keeping current with changing practices and interventions in patient care through lifelong learning.
13. Maintain a collaborative work relationship with peers and colleagues, creating a positive work environment.
14. Report pertinent observations and reactions regarding patients to the appropriate person and record those observations accurately and concisely.
Physical Requirements:
1. Prolonged, extensive, or considerable standing/walking.
2. Lifts, positions, pushes, and/or transfers patients and equipment.
3. Considerable reaching, stooping, bending, kneeling, crouching.
4. Frequent exposure to hazardous chemicals, sick patients, bodily substances, noise, and possible exposure to radiation, lasers, electric shock, etc.
5. Regularly exposed to the risk of bloodborne diseases and other transmissible infections.
6. Contact with patients under a wide variety of circumstances.
7. Subject to varying and unpredictable situations.
8. Handle emergency and crisis situations.
9. Subjected to irregular hours.
10. May have contact with hazardous materials.
Skills & Abilities:
1. Ability to effectively communicate with patients, families, physicians, and healthcare team members.
2. Knowledge and application of the nursing process to implement a nursing plan of care.
3. Possesses and applies the skills and knowledge necessary to provide care to patients throughout the life span, considering aging processes, human development stages, and cultural patterns in each step of the care process.
4. Demonstrates the skills and knowledge to provide care to the age groups of the population served, including knowledge of growth and development, ability to interpret age-specific data and response to care, and provide age-appropriate communication.
5. Demonstrates essential knowledge of pharmacology/medications in administering drugs and monitoring the patient's response.
