Job Summary

We are seeking a highly motivated and compassionate Patient Care Assistant to join our team. As a Patient Care Assistant, you will provide nursing care to patients under the supervision and direction of a registered nurse.

Responsibilities
  • Assist with daily living activities such as bathing, changing, toileting, dressing, and feeding patients.
  • Perform light housekeeping duties in patient rooms and maintain cleanliness in utility rooms, linen closets, and hallways.
  • Assist with clinical patient care tasks that do not require a nursing license, such as taking vital signs and assisting with medication administration.
  • Respond to call lights, answer phones, and orient patient families and visitors.
  • Maintain accurate and up-to-date electronic medical records and document all patient interactions.
  • Collaborate with the care team to review and discuss patient care plans and ongoing patient activities.
  • Ensure efficient patient flow by being aware of room availability and patient intakes and discharges.
  • Clean patient rooms after discharge and properly dispose of all items used during a patient stay.
Requirements
  • Must be at least 18 years of age.
  • Direct patient care experience preferred, but not required.
  • BLS certification required.
  • PA Nurse Aide certification required for Transitional Sub-Acute Unit (TSU) at Reading Health Rehabilitation Hospital (RHRH).
