Job Summary:

The Community Health Worker will play a pivotal role in providing follow-up support to Family Connects Nurse Home Visitors, ensuring the continuity and effectiveness of services provided to families. This position requires strong interpersonal skills, a passion for community health, and the ability to collaborate with various stakeholders.

Key Responsibilities:

Conduct home visits to assess the needs of families and offer assistance as necessary. Communicate with families via phone calls or in-person meetings to address any concerns or questions following Nurse Home Visits. Provide culturally sensitive support and host supplemental educational workshops for postpartum mothers, covering topics relevant to postpartum and newborn care. Connect families with community resources and services based on identified needs, including aid in accessing community resources, referrals to healthcare providers, social services agencies, childcare resources, and other relevant community resources. Maintain accurate and up-to-date records of interactions with families, including assessments, referrals, recruitment outcomes, and follow-up actions taken in accordance with the organization's policies and procedures. Work closely with the Family Connects team, including Nurse Home Visitors, program coordinators, and other staff members, to ensure seamless coordination of services and effective communication.

Community Outreach:

Participate in community outreach activities to promote awareness of Family Connects services and engage with potential clients. Participate in and contribute to the organization and execution of events aimed at fostering the growth and advancement of program services and partnerships. Assist with data collection and reporting on recruitment outcomes. Stay informed about best practices in community health, home visiting programs, and relevant resources through ongoing training and professional development opportunities.

Requirements:

Community Health Worker Certificate - preferred. Recent community Health work or at least two years of working with a health care provider or community-based organization in the area. Must be current on vaccinations and willing to provide immunization records. This position may require occasional evening or weekend hours to accommodate the needs of families. Travel within the community and Bexar County to conduct home visits and attend meetings/trainings is required. This could involve tasks such as transporting items/supplies to and from office locations, department stores, client's homes, or storage. This is a Grant funded position and salary is dependent on funding availability. Catholic Charities is an Equal Opportunity Employer.
