Nursing Care Coordinator

As a Registered Nurse Clinical Case Manager (RNCCM) at Providence Medical Group, you will play a vital role in delivering high-quality care to patients with complex medical and psychosocial issues. Our patient-centered medical home (PCMH) teams, led by physicians/providers, rely on your expertise to provide comprehensive care management and coordination services.
About the Role
This is a full-time position with a competitive salary range of $45.22 - $71.58 per hour. In addition to base pay, eligible employees may receive shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities. Key Responsibilities
- Provide care management and coordination services for at-risk patients
- Develop and implement individualized care plans based on patients' needs
- Collaborate with multidisciplinary teams to ensure seamless care transitions
- Apply population-based, chronic care management approaches and clinical protocols as needed
- Participate in quality improvement initiatives to enhance patient outcomes and experiences
Requirements
- Graduation from an accredited school of nursing
- Oregon Registered Nurse License upon hire
- 3 years of clinical experience as an RN
- Preferred qualifications: Bachelor's Degree in Nursing, Home health, skilled nursing facility, and/or community health nursing experience, Managed care experience and/or utilization management experience, Care/Case Management experience
