Job Description
The Clinical Eligibility Specialist for Community Programs determines clinical eligibility for MassHealth recipients/applicants requiring nursing facility care per regulations. This role promotes community service options that meet the consumer's wishes and needs for discharge. The specialist works with an interdisciplinary team to assess and assist consumers requesting to transition from the nursing facility back to the community.
Responsibilities:
- Determines clinical eligibility for MassHealth recipients/applicants for nursing facility care, adult day health, home- and community-based waiver services, and other community-based programs.
- Completes necessary documentation requirements according to Executive Office of Elder Affairs and MassHealth regulations.
- Coordinates discharge planning for elders transitioning from the nursing facility setting to the community.
- Performs on-site evaluations and clinical assessments of consumers in nursing facilities.
- Explores available community alternatives with consumers, involved supports, nursing facility staff, care managers, and options counselors to develop a service plan addressing the consumer's needs, values, and preferences.
Requirements:
- A valid Massachusetts Registered Nurse license is required. Applicants must have a valid driver's license and reliable transportation.
- Bachelor of Science in Nursing or Associate of Science in Nursing degree with at least one year of clinical experience in a community or long-term care setting; or Diploma RN with two additional years of community-based nursing experience or long-term care direct service experience.
- Ability to complete regular assessments and possess good written, verbal, interpersonal, and computer skills, as well as the ability to work in an interdisciplinary team-based environment.
