Job Overview

1st Choice, LLC is seeking a highly skilled Family Support Coordinator to join our team. As a key member of our organization, you will play a vital role in ensuring the success of our Children and Youth Evaluation Service (C-YES) program.

Key Responsibilities
  1. Outreach and Engagement
  • Conduct thorough outreach to gather consumer information and identify stakeholders involved in the consumer's care.
  • Perform timely outreach and achieve productivity targets related to scheduling assessment appointments with the appropriate individuals.
  • Document all outreach activities for tracking and reporting purposes.
Scheduling and Coordination
  • Schedule assessment appointments with appropriate resources, ensuring that the process runs efficiently and that consumer needs are met.
  • Assist in scheduling appointments with Registered Nurses and participate in appointments when possible.
Consumer Education and Inquiries
  • Respond to inquiries from stakeholders, providing education about the C-YES program and associated services.
  • Ensure that families and caregivers fully understand the services available to them.
Case Management and Support
  • Provide case management services to support the consumer's plan of care.
  • Identify and connect families with appropriate referrals and community resources that align with their care plan.
  • Follow up to ensure that consumers access community providers and resources as outlined in the care plan.
Program Compliance and Efficiency
  • Adhere to all work instructions, policies, and procedures related to the C-YES project.
  • Identify areas for process improvement and ensure that the scheduling process meets program objectives.
  • Meet all standards set forth for this position in the annual performance criteria and bonus template.
Other Duties
  • Perform other tasks as assigned by management to ensure project success.
Requirements
  • Education
    • Bachelor's Degree from an accredited college or university.
  • Experience
    • 1-3 years of experience in case management, specifically in a telephonic capacity.
  • Skills
    • Proficient in MS Excel and capable of quickly learning new software programs.
    • Strong organizational, interpersonal, written, and verbal communication skills.
    • Ability to work efficiently in a fast-paced, deadline-driven environment.
    • Able to multitask and work independently, as well as collaboratively within a team.
Preferred Qualifications
  • Education
    • Bachelor's Degree in Social Work or another mental health-related field from an accredited college or university.
  • Experience
    • Experience working with pediatric psychiatric and/or medically fragile patients.
  • Skills
    • Bilingual skills are preferred but not required (commonly spoken languages such as Spanish, Russian, or Haitian Creole are a plus).
Additional Information
  • Location: Hybrid position based in New York, with a combination of remote work and in-person responsibilities at the office or with clients as required.
  • Travel: Occasional travel may be required within New York State for in-person consumer support needs.
