Family Support Coordinator

1st Choice, LLC is seeking a highly skilled Family Support Coordinator to join our team. As a key member of our organization, you will play a vital role in ensuring the success of our Children and Youth Evaluation Service (C-YES) program.
Key Responsibilities- Outreach and Engagement
- Conduct thorough outreach to gather consumer information and identify stakeholders involved in the consumer's care.
- Perform timely outreach and achieve productivity targets related to scheduling assessment appointments with the appropriate individuals.
- Document all outreach activities for tracking and reporting purposes.
- Schedule assessment appointments with appropriate resources, ensuring that the process runs efficiently and that consumer needs are met.
- Assist in scheduling appointments with Registered Nurses and participate in appointments when possible.
- Respond to inquiries from stakeholders, providing education about the C-YES program and associated services.
- Ensure that families and caregivers fully understand the services available to them.
- Provide case management services to support the consumer's plan of care.
- Identify and connect families with appropriate referrals and community resources that align with their care plan.
- Follow up to ensure that consumers access community providers and resources as outlined in the care plan.
- Adhere to all work instructions, policies, and procedures related to the C-YES project.
- Identify areas for process improvement and ensure that the scheduling process meets program objectives.
- Meet all standards set forth for this position in the annual performance criteria and bonus template.
- Perform other tasks as assigned by management to ensure project success.
- Education
- Bachelor's Degree from an accredited college or university.
- Experience
- 1-3 years of experience in case management, specifically in a telephonic capacity.
- Skills
- Proficient in MS Excel and capable of quickly learning new software programs.
- Strong organizational, interpersonal, written, and verbal communication skills.
- Ability to work efficiently in a fast-paced, deadline-driven environment.
- Able to multitask and work independently, as well as collaboratively within a team.
- Education
- Bachelor's Degree in Social Work or another mental health-related field from an accredited college or university.
- Experience
- Experience working with pediatric psychiatric and/or medically fragile patients.
- Skills
- Bilingual skills are preferred but not required (commonly spoken languages such as Spanish, Russian, or Haitian Creole are a plus).
- Location: Hybrid position based in New York, with a combination of remote work and in-person responsibilities at the office or with clients as required.
- Travel: Occasional travel may be required within New York State for in-person consumer support needs.
