Job Summary

We are seeking an Advanced Nurse Practitioner to join our team in Taos, New Mexico. This part-time position involves conducting home risk assessments, medical history reviews, and providing education to patients and caregivers. The ideal candidate will have strong communication skills, be comfortable working in various settings, and have a passion for improving patient outcomes.

Key Responsibilities
  • Conduct comprehensive home risk assessments, including medical history reviews and observation of patient's current health
  • Provide education to patients and caregivers on disease management, medication adherence, and healthy lifestyle choices
  • Collaborate with physicians, case managers, and other healthcare professionals to ensure seamless care coordination
  • Work independently in a variety of settings, including home visits, skilled facilities, and telehealth
Requirements
  • Master's degree in nursing or equivalent experience
  • Current RN and NP licensure in the state of practice, including prescription authority
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
  • 1+ year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent verbal and written communication skills
