About the Role

We are seeking a skilled and compassionate Advanced Kidney Care Specialist to join our interdisciplinary clinical care team at Strive Health. As a vital member of our team, you will be responsible for delivering advanced nursing care to patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD).

Key Responsibilities

  • Manage patient care by working closely with our team of social workers, care coordinators, pharmacists, dietitians, and physicians to provide best-in-class patient care.
  • Be an advocate, navigator, trusted clinician, and safety net for patients while delivering exceptional clinical care and building empathy and trust with patients.
  • Presents patient cases and provides clinical support for clinical rounds and interdisciplinary team meetings.
  • Evaluate patient outcomes and adjust care plans as needed to ensure optimal results.

Requirements

  • A Master's degree in Nursing or a related field.
  • At least 2 years of experience as a Family Nurse Practitioner (NP), Primary Care Nurse Practitioner (NP), or experience in internal medicine, cardiology, nephrology, or case management.
  • Current state-specific licensure required.
  • National Board Certification.
  • Current DEA license required or eligible to obtain within 90 days of hire.

Benefits

  • An estimated annual salary range of $125,000 to $156,000, depending on experience.
