Job Summary:

As a Physical Therapy Assistant at Hackensack Meridian Health, you will be part of a dynamic team dedicated to delivering exceptional patient-centered physical rehabilitation services. Your role will involve supporting Physical Therapists in treatment procedures, ensuring the delivery of high-quality care to patients. This position requires strong communication and collaboration skills, as you will work closely with Physical Therapists to ensure the effective implementation of patient care plans.

Key Responsibilities:

  • Deliver patient care with minimal disruptions and obstacles, ensuring a seamless experience for patients.
  • Participate in multidisciplinary team meetings to discuss patient care plans and progress.
  • Assist with new Team Member orientation and provide key experiences to prepare Team Members for independent performance of essential job functions.
  • Comply with federal, state, and local legal and professional requirements by studying existing and new legislation, anticipating future legislation, enforcing adherence to requirements, and advising management on needed actions.
  • Identify and report any real or potential problems regarding the development or administration of patient care.
  • Assist with discharge planning by collaborating with physicians, nurses, social workers, supervising physical therapists, and other healthcare providers.
  • Contribute to patient care conferences.
  • Complete all documentation accurately and in a timely manner, consistent with all state regulations.

Requirements:

  • Completion of an accredited program and licensed as a Physical Therapist Assistant.
  • Current with all Continuing Education (CE) requirements.
  • Ability to effectively work on a multidisciplinary team.
  • Excellent written and verbal communication skills.
  • Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.

Preferred Qualifications:

  • Prior work experience with geriatric and disabled populations.

Licenses and Certifications Required:

  • Licensed Physical Therapy Assistant.
