Job Title: Licensed Vocational Nurse

We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our team at Prime Home Health. As an LVN, you will play a vital role in providing high-quality patient care in the comfort of patients' homes.

Job Summary:

The LVN will be responsible for providing direct patient care under the supervision of a registered nurse. This includes following the plan of care, providing treatments, and working collaboratively with the healthcare team to achieve positive patient outcomes.

Responsibilities:
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Implement the plan of care initiated by the registered nurse, ensuring that patients receive the highest level of care.
  • Document patient information accurately and timely, including progress notes and clinical notes.
  • Assess and educate patients and their families on disease management, medication adherence, and healthy lifestyle habits.
  • Collaborate with the healthcare team to ensure seamless care transitions and optimal patient outcomes.
Requirements:
  • Current California Vocational Nurse License.
  • Acceptance of new graduates under the Flex Waiver Program.
  • Current PPD or chest x-ray (if applicable).
  • Current State license, CPR certification, and valid driver's license (local travel required).
