Job Summary

We are seeking a highly skilled and compassionate Mental Health Nurse Specialist to join our team at NYC Health + Hospitals. As a key member of our healthcare team, you will play a vital role in providing high-quality care to patients with psychiatric needs.

About the Role

The ideal candidate will have a strong background in nursing and a passion for working with individuals with mental health conditions. You will be responsible for assessing patients' physical and psycho-social needs, developing individualized care plans, and providing direct care services in their homes and in the community.

Key Responsibilities
  • Conduct thorough assessments of patients' physical and psycho-social needs to develop comprehensive care plans.
  • Provide direct care services to patients in their homes and in the community, including medication management, therapy sessions, and case management.
  • Collaborate with interdisciplinary teams to ensure seamless coordination of patient care.
  • Maintain accurate and detailed records of patient progress and treatment plans.
Requirements

To be successful in this role, you will need:

  • A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State.
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
