About the Job

The Licensed Independent Social Worker will be part of a close-knit team to improve health care outcomes and support clients taking their medications as prescribed. You will work directly with patients and healthcare providers to ensure coordination of services and meet the unique needs of people living with HIV.

In this role, you will build rapport, provide supportive listening, and educate patients about their diagnoses and treatment options. You will also develop individualized care plans and provide ongoing education to patients so they understand their medications.

For Ryan White contracts that combine psychosocial needs and resources within Medical Care Management, you will identify mental health conditions or substance abuse patterns that interfere with adherence to medical/medication regimens and motivate patients to access mental health or substance abuse treatment services.

Required Skills and Qualifications
  • LISW required (current and in good standing)
  • 1 Year minimum of Case Management experience preferred
  • Knowledge of Ryan White Services preferred
