We are seeking a highly motivated and dedicated Community Health Worker to join our team at the University of Arkansas for Medical Sciences. The successful candidate will be responsible for managing relationships with patients, local community-based organizations, and community members to provide education, resources, and health education.
Key Responsibilities- Conduct outreach to individuals, communities, service providers, and groups to identify and recruit individuals, families, and community groups to services and systems.
- Educate health and social service systems and providers about community perspectives and cultural norms.
- Facilitate cross-cultural communication among individuals, communities, and health/social service system workers.
- Coordinate care, provide case management support, and assist individuals and communities in navigating health and social service systems.
- Participate in the design, implementation, and interpretation of community-level assessments and present at local agencies and community events to share information and educate individuals and communities about health and social service concerns and resources.
- High School Diploma/GED plus two (2) years of general experience in community outreach, education, case management, and administration experience.
- Must enroll in a certified CHW Training Program within 45 days of employment.
- Bachelor's Degree or High School Diploma/GED plus four (4) years of general experience in community outreach, education, case management, administration experience.
- Completed certified CHW training program.
- Mastered all CHW Competencies for Level I.
