Physical Therapist Assistant

We are seeking a skilled Physical Therapist Assistant to join our team at Tufts Medicine Care at Home. As a key member of our interdisciplinary team, you will work closely with licensed Physical Therapists to provide high-quality rehabilitative care to patients in various settings.
Key Responsibilities- Provide patient care and services as planned and delegated by the Physical Therapist, in accordance with the plan of care.
- Document services in compliance with agency policies and professional standards.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in IDT meetings and contribute to the development of individualized care plans.
- Ensure compliance with regulatory requirements and Tufts Medicine policies and procedures.
