Key Responsibilities
  • Coordinate patient referrals to specialty providers, ensuring timely and efficient communication.
  • Assemble and provide clinical information to specialists, adhering to referral guidelines.
  • Communicate with specialty offices to schedule appointments and ensure prior approval requirements are met.
  • Organize and track referrals, maintaining accurate documentation in the electronic health record.
  • Review referral details and expectations with patients, including financial obligations.
  • Act as a resource for patients and providers, addressing questions and concerns related to referrals.
  • Establish and maintain relationships with service providers, ensuring timely and effective referrals.
  • Prioritize referrals, ensuring urgent items are addressed promptly.
  • Collaborate with the outreach team to assist patients with enrollment and follow-up assistance for ACA insurance applications.
  • Participate in quality improvement activities, contributing to monitoring, measuring, and analyzing program goals and services.
Requirements
  • 2 years of related work experience preferred.
  • Knowledge of medical terminology, Microsoft Office, and local insurance carriers and plans.
  • Excellent verbal and written communication skills, with bilingual skills in English and Spanish preferred.
  • Certified Application Counselor certification required within 6 months of hire.
