We are seeking a compassionate and skilled RN Case Manager to join our team at Commonwealth Hospice in Martinsville, VA. As a Hospice RN, you will have the opportunity to develop trusting relationships with patients and their families, providing end-of-life care with empathy and compassion.
Key responsibilities include:
- Conducting initial nursing evaluations to determine eligibility for hospice services
- Identifying patients' physical, psychosocial, and environmental needs, and re-assessing as needed
- Initiating and coordinating the plan of care, maintaining accurate and relevant clinical notes
- Collaborating with patients, families, attending physicians, and other members of the IDG to provide daily care
Requirements include:
- One year of clinical experience
- Current RN licensure in the state of practice and one year of clinical experience
- Current CPR Certification
- Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation
