We are seeking a compassionate and skilled RN Case Manager to join our team at Commonwealth Hospice in Martinsville, VA. As a Hospice RN, you will have the opportunity to develop trusting relationships with patients and their families, providing end-of-life care with empathy and compassion.

Key responsibilities include:

  • Conducting initial nursing evaluations to determine eligibility for hospice services
  • Identifying patients' physical, psychosocial, and environmental needs, and re-assessing as needed
  • Initiating and coordinating the plan of care, maintaining accurate and relevant clinical notes
  • Collaborating with patients, families, attending physicians, and other members of the IDG to provide daily care

Requirements include:

  • One year of clinical experience
  • Current RN licensure in the state of practice and one year of clinical experience
  • Current CPR Certification
  • Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation
