The Caregiver Coordinator is responsible for delivering exceptional care services to residents in accordance with their individualized plans. Key duties include:
- Providing medication management services, including passing medications as prescribed by physicians;
- Coordinating daily routines and activities, ensuring resident safety and well-being;
- Maintaining accurate records of resident care, including documentation of tasks delivered and observed changes in condition;
- Collaborating with healthcare professionals to ensure seamless care transitions;
- Leading by example, demonstrating excellent communication and interpersonal skills.

- Competitive salary range $42,000 - $50,000 per year, depending on experience;

To be successful in this role, you'll need:
- A high school diploma or equivalent;
- At least one year of caregiving experience or a similar function;
- State required HHA certification or CNA license;
- Supervisory experience in a health-related field (six months or more);
- Excellent communication and organizational skills.
