The Caregiver Coordinator is responsible for delivering exceptional care services to residents in accordance with their individualized plans. Key duties include:

• Providing medication management services, including passing medications as prescribed by physicians;
• Coordinating daily routines and activities, ensuring resident safety and well-being;
• Maintaining accurate records of resident care, including documentation of tasks delivered and observed changes in condition;
• Collaborating with healthcare professionals to ensure seamless care transitions;
• Leading by example, demonstrating excellent communication and interpersonal skills.

To be successful in this role, you'll need:

• A high school diploma or equivalent;
• At least one year of caregiving experience or a similar function;
• State required HHA certification or CNA license;
• Supervisory experience in a health-related field (six months or more);
• Excellent communication and organizational skills.
