Job Summary:The Staff Nurse will conduct thorough healthcare assessments of clients, providing medical expertise to support APS caseworkers and supervisors on complex cases. You will also complete in-home healthcare evaluations, make linkages to appropriate healthcare providers, and participate in Multi-Disciplinary Teams to impart medical expertise in critical cases.
Key Responsibilities:- Conduct in-home healthcare assessments, including physical health, mental status, and cognitive skills.
- Refer clients who are medically non-compliant to appropriate providers and social work staff.
- Evaluate signs of abuse and make the appropriate medical referrals.
- Collaborate with APS caseworkers and external entities to arrange appropriate hospitalization.
- Evaluate client's abilities to perform activities of daily living and the degree to which the client requires assistance.
- Identify client's medical needs and outline goals and interventions to achieve wellness.
- Develop a plan of care for the type of home care service to be provided to the client.
- Gather information for medical histories and complete medication reviews, consulting with medical providers when necessary.
- Conduct assessments of clients transitioning to a Nursing Home Facility and complete the PRI (Patient Review Instrument) for placement in long-term care facilities.
- Provide in-service training in healthcare issues for APS caseworkers and supervisors.
- A valid New York State Registered Nurse License.
