AOM Healthcare is a leading provider of long-term care services, operating 21 facilities across Ohio. We are currently seeking a Full-Time Licensed Practical Nurse (LPN) to join our team at Concord Care of Hartford.

About the Role

We are looking for a skilled and compassionate LPN to provide high-quality patient care on our night shift. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our residents and their families.

Responsibilities
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring
  • Collaborate with the healthcare team to develop and implement individualized care plans
  • Conduct thorough assessments and document patient information accurately
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments
Requirements

To be successful in this role, you will need:

  • A graduate degree in Licensed Practical Nursing from an accredited program
  • Previous long-term care experience, with a focus on LPN roles
  • Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare teams
  • Ability to meet health assessment requirements, including a two-stage Mantoux skin test
  • Current licensure as an LPN in the state of Ohio
