Advanced Practice Nurse for Home and Community Care

At Optum, we're creating a new model of care that addresses the physical, mental, and social needs of our patients. We're seeking an experienced Advanced Practice Nurse to join our team in New Hampshire.

About the Role:

This is a per diem opportunity to provide medical care services to Optum members in their homes or nursing facilities. As a member of our Senior Community Care (SCC) product, you'll work with a team to deliver cost-effective, quality care to assigned members.

Responsibilities:

  • Primary Care Delivery: Deliver comprehensive assessments, manage both medical and behavioral conditions, and document findings in a concise manner. Ensure diagnoses are ICD10 coded accurately and documented appropriately.
  • Care Coordination: Understand payer/plan benefits, CCM associate policies, and articulate them effectively to providers and members. Assess medical necessity/effectiveness of ancillary services and coordinate care during transitions.
  • Program Enhancement Expected Behaviors: Regularly communicate with internal and external parties, promote the CCM program, and exhibit original thinking and creativity in developing new approaches.

Requirements:

  • Certified Nurse Practitioner through a national board (AANP or ANCC)
  • Active and unrestricted license in New Hampshire
  • Current active DEA licensure/prescriptive authority
  • Access to reliable transportation
  • Ability to lift 30 pounds and navigate stairs/dwelling conditions
  • Availability to work 24 hours/month, including off-hours (up to 960 hours/year)

Preferred Qualifications:

  • 1+ years of hands-on post-grad experience in Long Term Care
  • Understanding of Geriatrics and Chronic Illness
  • Proficient computer skills, including electronic medical records

Salary Range:$83,600 - $111,400 per year, depending on experience.
