Healthcare Professional Wanted for RN Admissions Role

Seeking an experienced Registered Nurse to join our team at Robinson Terrace Rehabilitation and Nursing Center. This exciting opportunity involves coordinating the admissions process, collaborating with healthcare professionals, and ensuring exceptional patient care.

We are a 120-bed long-term and short-term facility providing premium healthcare services in Stamford, New York. Our dedicated staff is committed to delivering state-of-the-art care, exceeding resident expectations, and fostering a supportive community environment.
Job Overview:
This RN Admissions role requires the ability to work independently, assess residents' needs, and develop effective care plans. The successful candidate will have excellent communication skills, be able to make informed decisions, and demonstrate strong leadership abilities.
Essential Responsibilities:
- Initiate admission assessments, create initial care plans, and collaborate with the interdisciplinary team
- Participate in discharge planning, review and evaluate resident care plans as necessary
- Complete required charting, maintain confidentiality of resident information, and perform audits to ensure compliance with facility standards
- Supervise, direct, and provide resident care as needed, utilizing expertise in clinical assessment and decision-making
Necessary Skills and Qualifications:
- Demonstrate excellent clinical assessment skills, ability to make informed decisions, and strong leadership abilities
- Possess computer proficiency in MS Office (Excel, Word, Outlook) and have a valid NYS Registered Nurse License
- Have long-term care experience preferred, but not required
