About the Role

Gentiva Hospice is seeking a compassionate and skilled Hospice Registered Nurse / RN Case Manager to join our team. As a member of our interdisciplinary team, you will provide high-quality patient care and support to patients and their families in their homes.

Key Responsibilities
  • Provide direct patient care services to hospice patients and supportive care to the patient's family.
  • Act as primary liaison between physician, patient/family and hospice team.
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient.
  • Maintain all assigned patient clinical records.
  • Participate in company quality assessment and performance improvement programs.
Requirements
  • RN license in the state of requested employment (or eligibility to obtain).
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
