Job Summary

We are seeking a highly skilled and dedicated Registered Nurse to join our team at Kaiser Permanente. As a Lead Health Care Professional, you will be responsible for providing professional nursing care to patients, utilizing the nursing process in accordance with established standards of care, policies, and procedures.

As a key member of our health care team, you will demonstrate performance consistent with the Medical Center Vision, Mission, and Strategic Plan of the organization. You will remain flexible to changing systems and demonstrate quality and effectiveness in work habits and clinical practice.

You will treat co-workers, patients, families, and all members of the health care team with dignity and respect. Your role will involve serving as a leader of the health care team, delegating tasks appropriately, and demonstrating appropriate accountability.

Essential Responsibilities
  • Leadership:
    • You will uphold Kaiser Permanentes Policies and Procedures, Principles of Responsibilities, and applicable state, federal and local laws.
      • You will serve as a leader of the health care team; delegate tasks appropriately, and demonstrate appropriate accountability.
        • You will understand own and team members scope of practice and escalate issues as appropriate.
          • You will demonstrate professional, supportive behavior.
            • You will champion new ideas.
              • You will lead and direct others through the change process.
                • You will utilize communication strategies including chain of command and issue escalation, which result in intended outcomes.
                  • You will participate in problem identification and resolution.
                    • You will mentor, orient, and coach others in unit specific operations and patient care activities.
                      • You will share responsibility and authority with subordinates and hold him/her accountable for performance.
                        • You will demonstrate ability to problem solve with other departments in order to assist member problem resolution.
                          • You will prioritize, delegate, and supervise work assignments appropriately to ensure completion of patient care activities.
                            • You will comply with regulatory requirements, policies, procedures, and standards of practice.
                            Nursing Process
                            • You will develop and/or contribute to the individualized plan of care that reflects assessment, planning, implementing, and evaluating the outcomes of that plan.
                              • You will ensure plan shows multidisciplinary planning, consultation, and education.
                                • You will ensure plan is reflective of admission or outpatient database, on-going findings, age appropriate care, cultural specific needs, and appropriate acuity.
                                  • You will ensure plan is discussed with patient, family/significant others, and completed in a timely manner.
                                    • You will monitor the patients progress based on the plan.
                                      • You will revise plan on ongoing basis based on patient condition and evaluation of progress.
                                        • You will ensure care meets standards of practice.
                                          • You will ensure effective development and completion of discharge plan including discharge barriers and patient/family education. Ensure that patient clearly understands discharge instructions. In outpatient, identify barriers and needs for patient/family education that will facilitate the outpatient medical management plan.
                                            • You will ensure patient safety related but not limited to, medications and procedures utilizing the five rights; patient falls; decubitus prevention and prevention of nosocomial infections.
                                              • You will ensure optimal pain control and patient comfort; identifies and discusses patient anxieties, fears or concerns regarding patient condition, treatment or discharge.
                                                • You will ensure that patient understands medication purpose, side effects, and administration instructions in the hospital as well as at the time of discharge.
                                                Documentation
                                                • You will chart accurately, legibly, dated, and timed.
                                                  • Your documentation will reflect nursing process and interventions and evaluations taken.
                                                    • You will utilize computer systems effectively and efficiently for optimal patient care.
