Innovative Home-Based Physical Therapist Opportunities

Rehab Without Walls is a pioneering neuro rehabilitation program that empowers individuals to regain functional capabilities beyond the confines of institutional settings. By providing care in the home, community, workplace, or school, our revolutionary approach promotes greater health through the recovery process.
We specialize in helping individuals who have primarily experienced a brain injury, spinal cord injury, or stroke/CVA due to accident or illness. Our goal is to significantly improve functional skills and independence for patients.
What We're Looking For- A skilled PT with experience in home or community settings, particularly those with neurological conditions.
- An active license in the state of California is required.
As a Physical Therapist with Rehab Without Walls, your responsibilities will include:
- Performing evaluations and developing treatment plans tailored to each patient's unique needs.
- Regularly reassessing the effectiveness of treatment plans and attending staffing meetings, family conferences, and discharge evaluations.
- Making appropriate recommendations for home and community safety, providing referral and education resources as needed.
- Communicating patient progress to the treatment team, physician, person receiving services, and family members.
- Collaborating with other disciplines to ensure seamless care coordination and enhance patient outcomes.
- A minimum of one year's work experience as a physical therapist preferred.
- Demonstrates knowledge of rehabilitation techniques related to complex neurological injuries.
- Effective communication and professional verbal and written interactions are essential.
- The ability to lift 50 pounds and moving, lifting, or transferring patients safely.
