Patient Care Coordinator Specialist

The Patient Care Coordinator Specialist plays a pivotal role at Griffin Health Services Corporation, ensuring seamless navigation through the healthcare system for patients, practitioners, and hospital employees. This position is crucial in supporting care management activities and readmission prevention initiatives.
Main Responsibilities:
- Clinical Support: Collaborate with ACO clinical staff to perform various administrative duties that support the Outpatient Care Coordination department's objectives.
- Patient Navigation: Assist patients, practitioners, and hospital employees in navigating the healthcare system, providing critical support for care management activities and readmission prevention initiatives.
- Resource Identification: Work closely with teams to identify available resources for beneficiaries, ensuring optimal care outcomes.
Requirements:
- Education: LPN or RN License in CT required.
- Experience: Minimum 3 years of nursing experience in an acute care, post-acute, or home health capacity, demonstrating knowledge of regulatory guidelines and proficiency in identifying measurable functional rehab goals.
- Skills: Excellent interpersonal skills, ability to work collaboratively as a team member, and proficiency in using Microsoft Excel.
Estimated Salary: $65,000 - $80,000 per year, depending on location and experience.
