Job Summary

We are seeking a skilled Care Manager to join our team at Triangle Area Network Inc. As a Care Manager, you will play a vital role in enhancing patient-centered care by working with clinical care teams to engage patients and connect them to community resources.

Key Responsibilities
  • Accurately describe care management services to patients and encourage their participation.
  • Participate in outreach events to raise community awareness for new and existing patients.
  • Meet with patients in person and on the phone to assist with gaining access to community resources, including education, patient assistance, transportation, insurance, and more.
  • Link patients to specialty disease sets, including infectious diseases.
  • Evaluate the effectiveness of resources and programs for patients' benefit.
  • Schedule and confirm appointments, collect patient data, and document all patient interactions and encounters in the EMR.
  • Assist patients with Market Place (ACA), Medicaid, and Medicare insurance enrollment.
  • Document all patient interactions and encounters in the EMR.
  • Enhance patient connection to PCMH (Patient-centered medical home).
  • Effectively implement crisis intervention and problem-solving skills as needed.
  • Ability to work both independently and within a team environment.
Requirements
  • Ability to pass a background check.
  • An associate degree, preferably a bachelor's in a health-related field.
  • Ability to successfully pass a background check and pre-employment physical health exam.
  • At least one year of experience working in the healthcare field.
  • Proficient in Microsoft software, windows-based computer applications, and online research.
  • Excellent oral and written communication skills and the ability to carry out written and oral instructions.
  • Understanding of and the ability to work with current communities or demographics served by the health centers.
  • Detail-oriented, organized, and flexible.
