Job Title: Nurse Navigator

UPMC is seeking a dedicated and compassionate Nurse Navigator to join our women's health specialty services in the Horizon-Hermitage area. This full-time position will be part of our multidisciplinary team at The Pregnancy and Women's Recovery Center, providing comprehensive care to women with substance use disorder.

Responsibilities:
  • Collaborate with the interdisciplinary team to assess, plan, intervene, evaluate, and document care for patients.
  • Provide emotional support, specialized patient care, and educational needs to patients and their families.
  • Assume the role of care coordinator and patient advocate throughout the process.
  • Use the nursing process to provide comprehensive nursing care, support, and education to patients and their families.
  • Work closely with the interdisciplinary team to meet the education and development needs of professional and ancillary staff.
  • Integrate current research findings and technology into practice, performance improvement, and education as appropriate.
  • Participate in the evaluation of patient care and programs through quality improvement activities.
  • Monitor changes in legislation and regulations to ensure timely and consistent actions.
Requirements:
  • Registered Nurse currently licensed in the practicing state.
  • Current certification in specialty or obtained within one year.
  • CPR certification based on AHA standards.
  • Three to five years of current clinical experience required.
  • Preferred experience with the population served (i.e. women's health, breast health, oncology, etc.).
