Ambulatory Care Nurse



At St. Clair Health, we are seeking a skilled Ambulatory Care Nurse to join our team. As an Ambulatory Care Nurse, you will be responsible for planning, organizing, and directing patient care in accordance with established standards and skill competency.



Key Responsibilities:

  • Provide high-quality patient care in accordance with established standards and skill competency.

  • Develop and implement individualized patient care plans in collaboration with the healthcare team.
    \t
  • Conduct thorough assessments, identify patient needs, and develop effective care plans.
    \t
  • Collaborate with the healthcare team to ensure seamless communication and coordination of care.
    \t
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
    \t
  • Maintain accurate and up-to-date patient records in accordance with regulatory requirements.
    \t
  • Stay current with industry developments, best practices, and advancements in patient care.
    \t
  • Contribute to a positive and supportive work environment that promotes teamwork and collaboration.
    \t
  • Participate in ongoing education and training to enhance clinical skills and knowledge.
    \t
  • Adhere to St. Clair Health's policies and procedures, as well as regulatory requirements and industry standards.
    \t
  • Perform other duties as assigned by management.
    \t
  • Current Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), and Pediatric Advanced Life Support (PALS) certifications or currently scheduled for certification.
    \t
  • Ability to successfully complete a competency-based orientation and annual competency testing of skills and/or knowledge necessary to provide care to specific patient populations.

Requirements:




  • \t
  • Must possess a current license or a temporary practice permit to practice professional nursing in the Commonwealth of Pennsylvania.
    \t
  • Appropriate clinical experience with a thorough knowledge of the scientific principles of nursing and professional nursing standards, developed skill in delivering nursing care, and the ability to adapt to individual needs.
    \t
  • A fundamental understanding of personal computers and familiarity with the Windows operating system.
    \t
  • The ability to successfully complete a competency-based orientation and annual competency testing of skills and/or knowledge necessary to provide care to specific patient populations.
    \t
  • Current Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), and Pediatric Advanced Life Support (PALS) certifications or currently scheduled for certification.
    \t
  • One to three years of clinical experience in a related field, or equivalent combination of education and experience.
    \t
  • Bachelor's degree in Nursing or equivalent degree required, or currently pursuing a degree in Nursing with completion of degree within six months of hire date.
    \t
  • One year of critical care experience, or equivalent experience in a related field.

Preferred Qualifications:

  • One to three years of clinical experience in a related field, or equivalent combination of education and experience.
