Job Summary:

The Tampa General Hospital is seeking a highly skilled Advanced Heart Failure Care Coordinator to join our team. As a key member of the interdisciplinary team, you will play a crucial role in implementing care plans and treatments for patients with advanced heart failure.

In this rewarding position, you will work closely with physicians and other healthcare professionals to ensure that patients receive the highest level of care. Your primary responsibility will be to review medical records, anticipate clinical stability, and develop a thorough understanding of each patient's needs prior to follow-up consultations. You will also work to identify effective strategies to address barriers, reduce chronic disease severity, and prevent readmissions.

To excel in this role, you must possess excellent communication and collaboration skills, as well as the ability to escalate barriers through appropriate channels. Additionally, you will work in partnership with physicians to consider alternative levels of care when patients are not meeting clinical stability. Collaborating with Social Work, you will manage high-acuity psychosocial needs while providing medical care to mitigate risk for readmission.

A key component of this position is performing job duties in accordance with the hospital's mission, vision, and values. This includes working effectively with patients to assist them in self-managing their chronic diseases and adopting lifestyle changes to mitigate health risks.

Required Skills and Qualifications:

* Current RN license from the State of Florida
* Minimum 2 years of experience in acute care or ambulatory settings OR current certification in Cardiac/Heart Failure
