Job Overview:

We are seeking a skilled Patient Transition Specialist to join our team at Tenet Healthcare Corporation in Worcester, MA. As a key member of our healthcare team, you will play a vital role in ensuring seamless transitions for patients across the continuum of care.

**Salary:** $63,000 - $93,000 per year, depending on experience

About the Role:

The Patient Transition Specialist is responsible for facilitating effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources. This involves assessing patients' transition needs, identifying those at risk for readmission, and conducting complex psycho-social assessments and interventions to promote timely throughput, safe discharge, and prevent avoidable readmissions.

Key responsibilities include:

• Conducting comprehensive psycho-social assessments and developing individualized transition plans
• Coordinating with multidisciplinary teams to ensure patient-centered care
• Collaborating with physicians, office staff, and ancillary departments to promote efficient care delivery
• Providing education to patients, families, and caregivers to support post-acute needs
• Maintaining accurate patient demographic and insurance information
• Participating in quality improvement initiatives to enhance patient outcomes

Requirements:

To be successful in this role, you will need a Master's degree in Social Work (MSW) and relevant experience in acute hospital settings. A strong understanding of case management principles and practices is essential. Additionally, you must be licensed, certified, or registered to practice as a social worker in the state of Massachusetts. Preferred qualifications include Accredited Case Manager (ACM) certification.
