Youth Advocacy and Well-Being Specialist

Key Responsibilities:
- Develop and implement individualized care plans for youth and families, addressing medical, social, educational, and behavioral health needs.
- Advocate for youth and families with behavioral health, educational, and community partners.
- Coordinate referrals to services and related agencies to meet identified needs.
- Establish and maintain natural, community, and professional supports to aid youth and families in thriving in their communities.
- Provide educational sessions and training for families, community partners, and UTHSC staff.
- Complete referrals and enter notes in the case management system.
- Provide follow-up to youth, families, and agencies.
- Adhere to HIPAA guidelines and maintain confidentiality.
Requirements:
- High School Diploma or GED.
- Two years of related experience or Associate's Degree in lieu of experience.
- Valid Driver's License and vehicle insurance.
Preferred Qualifications:
- Knowledge of community resources to assist youth and families.
- Experience in fostering community partnerships and collaborations.
- Skills working with youth and families who have behavioral health concerns.
- Ability to relate to youth and families.
- Ability to maintain confidential information.
Work Schedule: This position may occasionally require working weekends and evenings, with occasional overnight travel.
