Hospice Nurse Manager

The Hospice Nurse Manager will play a vital part in our team at Advocate Aurora Health, responsible for overseeing the delivery of high-quality care to patients and their families. This role requires strong leadership skills, excellent communication abilities, and a deep understanding of hospice care principles.
Key Responsibilities:
- Lead interdisciplinary teams to develop and implement patient care plans that meet individual needs.
- Collaborate with healthcare professionals to ensure seamless transitions of care and efficient resource allocation.
- Conduct regular assessments to identify areas for improvement and implement necessary changes.
- Foster open communication among staff, patients, and families to promote empathy and understanding.
- Maintain accurate records and documentation to meet regulatory requirements.
Requirements:
- Registered Nurse license issued by the state in which you practice.
- Basic Life Support (BLS) certification from the American Heart Association.
- Minimum 1-2 years of experience in nursing, preferably in hospice or palliative care settings.
- Excellent leadership, communication, and critical thinking skills.
- Able to work effectively in a fast-paced environment with multiple priorities.
