We are seeking a compassionate and skilled Nurse Practitioner to join our team at Abode Care Partners. Our company is dedicated to providing high-quality medical care to seniors and patients with complex needs in the comfort of their own homes or assisted living facilities.

The ideal candidate will have strong interpersonal communication skills, excellent time management skills, and the ability to work independently. Additionally, they will be proficient in medical instruments and equipment, as well as computer-based data management programs and electronic medical record systems.

This role offers a competitive salary of $115,000 per year, along with opportunities for professional growth and development. About the Role:

  • Perform evidence-based medical assessments and treatments
  • Collaborate with healthcare professionals to provide interdisciplinary care
  • Maintain accurate and up-to-date patient medical records
  • Provide referrals to other healthcare practitioners as needed

Required Skills and Qualifications:

  • Active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
