Job Summary

At St Croix Hospice, we are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice Care Nurse. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality, patient-centered care to individuals and their families in their homes and communities.

Responsibilities
  • Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
  • Provide professional nursing care, including administering medications, treatments, and other interventions as prescribed by physicians.
  • Counsel patients and families on meeting their nursing and related needs, and provide health care instructions as appropriate.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in quality assessment and performance improvement activities to ensure the highest level of care.

Requirements
  • Graduate of an accredited school of nursing with a current licensure in the state.
  • One to two years of experience in hospice care, with a preference for those with management experience.
  • Excellent observation, verbal and written communication skills, problem-solving skills, and basic math skills.
