Caregiver Specialist

Ohio's Hospice is seeking a compassionate and dedicated Personal Care Specialist to join our team. As a valued member of our care team, you will provide exceptional support to patients and their families, ensuring their comfort and dignity throughout their end-of-life journey.
Key Responsibilities- Provide personalized care and support to patients, including assistance with daily living activities, medication management, and emotional support.
- Collaborate with healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.
- Communicate effectively with patients, families, and caregivers to ensure seamless care coordination and address any concerns or questions.
- Participate in ongoing education and training to enhance your skills and knowledge in end-of-life care and hospice services.
- High school diploma or equivalent required; one year of experience in healthcare or a related field preferred.
- Certified State Tested Nursing Assistant (STNA) certification in the State of Ohio required.
- Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
- Valid driver's license and reliable transportation required.
