Nursing Professional for Community Living

ResCare Community Living is a leading provider of disability services, offering high-quality support to individuals with intellectual, developmental, or cognitive disabilities. As a Licensed Practical Nurse (LPN) in our team, you will play a vital role in delivering exceptional care and support to our clients.
About the Role:
We are seeking an experienced LPN to join our team in providing routine care, general observation of client health status, and assisting registered nurses and physicians in treatments. Your primary responsibility will be to communicate effectively with clients, their families, and healthcare professionals, ensuring seamless coordination of care.
Key Responsibilities:
- Communicate health and nursing needs to registered nurse supervisors
- Coordinate admissions, discharges, and follow-up as needed
- Assist in medical appointments as required
- Ensure provision of nursing services under the direction of registered nurses and physicians
- Take and record vital signs, weights, wound measurements, and other essential data
- Report pertinent information and changes in individual health status to attending physicians and healthcare professionals
- Provide necessary adaptive equipment and ensure accurate documentation
- Administer medications according to established protocols
- Work closely with home, facility, and pharmacy staff in medication management
- Participate in quality improvement activities, team meetings, and interdisciplinary teams
- Adhere to company policies and procedures
Requirements:
- Completion of a state-approved training program
- Licensed Practical/Vocational Nurse (LPN or LVN) in good standing
- CPR certification
- At least 6 months of direct nursing experience
- Experience in working with individuals with developmental disabilities preferred
- Valid driver's license with a satisfactory driving record
