Your expertise will be essential in developing and implementing comprehensive care plans that address the unique needs of each patient.

Key Responsibilities:

  • Provide skilled nursing care to patients in their homes, including admission, case management, and follow-up visits.
  • Confer with physicians to develop and revise care plans based on ongoing assessments and patient progress.
  • Coordinate care with other healthcare professionals, including physical therapists, occupational therapists, and home health aides.
  • Monitor patient progress, report to physicians and Clinical Managers, and make adjustments to care plans as needed.
  • Supervise and oversee the work of home health aides and personal care workers, ensuring that care plans are implemented effectively.

Requirements:

  • Diploma, Associate, or Bachelor's degree in Nursing.
  • Minimum of one year of nursing experience, preferably in home health or a related field.
  • Strong med-surg, ICU, ER, and acute care experience.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Strong organizational and communication skills.
  • Valid driver's license, auto insurance, and reliable transportation.
