About the Role:
As a Home Health Nurse Specialist with CenterWell Home Health, you will play a vital role in delivering high-quality, patient-centered care to individuals in their homes. Your expertise will be essential in developing and implementing comprehensive care plans that address the unique needs of each patient.
Key Responsibilities:
- Provide skilled nursing care to patients in their homes, including admission, case management, and follow-up visits.
- Confer with physicians to develop and revise care plans based on ongoing assessments and patient progress.
- Coordinate care with other healthcare professionals, including physical therapists, occupational therapists, and home health aides.
- Monitor patient progress, report to physicians and Clinical Managers, and make adjustments to care plans as needed.
- Supervise and oversee the work of home health aides and personal care workers, ensuring that care plans are implemented effectively.
Requirements:
- Diploma, Associate, or Bachelor's degree in Nursing.
- Minimum of one year of nursing experience, preferably in home health or a related field.
- Strong med-surg, ICU, ER, and acute care experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- Valid driver's license, auto insurance, and reliable transportation.
