Responsibilities:
  • Conduct member outreach to North Carolina members who are overdue for preventive care services and screenings.
  • Engage with members, parents, or guardians to address social determinants of health, connect them to resources, and promote healthy outcomes.
  • Conduct screenings of member healthcare needs, work with members to develop healthcare goals, and identify potential barriers to achieving those goals.
  • Utilize motivational interviewing techniques to help members identify and understand their intrinsic goals and motivate them to engage in positive behavior change.
  • Coordinate member access to health-related programs, services, and providers.
  • Coordinate with healthcare providers to facilitate member care and services.
  • Provide education to members on health and wellness information, member plan benefits, and how to access those benefits.
  • Review member health-related information using internal and external systems.
  • Use desktop computer applications (Microsoft Excel, Word, Outlook) to summarize information or communicate with others.
  • Meet performance goals within a virtual environment.
Qualifications:
  • At least 3 years of experience in a community health setting in a healthcare role.
  • High school diploma or GED.
  • Proficiency in Microsoft Office applications.
  • Excellent problem-solving, prioritization, and organization skills.
