Responsibilities:- Conduct member outreach to North Carolina members who are overdue for preventive care services and screenings.
- Engage with members, parents, or guardians to address social determinants of health, connect them to resources, and promote healthy outcomes.
- Conduct screenings of member healthcare needs, work with members to develop healthcare goals, and identify potential barriers to achieving those goals.
- Utilize motivational interviewing techniques to help members identify and understand their intrinsic goals and motivate them to engage in positive behavior change.
- Coordinate member access to health-related programs, services, and providers.
- Coordinate with healthcare providers to facilitate member care and services.
- Provide education to members on health and wellness information, member plan benefits, and how to access those benefits.
- Review member health-related information using internal and external systems.
- Use desktop computer applications (Microsoft Excel, Word, Outlook) to summarize information or communicate with others.
- Meet performance goals within a virtual environment.
- At least 3 years of experience in a community health setting in a healthcare role.
- High school diploma or GED.
- Proficiency in Microsoft Office applications.
- Excellent problem-solving, prioritization, and organization skills.
