Job Summary

The RN Navigator Pulmonology, Critical Care, and Sleep will work independently to plan, implement, and evaluate nursing care in a patient care ambulatory setting. This role will provide direct nursing care to patients, assume teaching and leadership responsibilities, and complete CPAP mask fittings and teaching for patients.

Key Responsibilities
  • Coordinate the referral process and introduction of patients into the PNC, ILD, and Sleep Programs
  • Employ patient teaching to promote health and safety
  • Evaluate progress toward outcomes
  • Practice ethically
  • Receive incoming referrals from physicians and hospitals and respond quickly
  • Triage referrals according to written criteria
  • Communicate back to referring physicians and hospitals the plans for acceptance into the program
  • Educate patients and families regarding the referral process and keep them informed throughout the process
  • Demonstrate leadership in the professional practice setting
  • Collaborate with patients, families, and the inter-professional team
  • Utilize appropriate resources to plan and provide nursing services
  • Practice in an environmentally safe and healthy manner
  • Work in collaboration with providers and clinic staff to ensure a smooth transition of patients
  • Oversee Pulmonary Lung Nodule Clinic (PNC) screenings and coordinate free screening CTs
  • Oversee PNC and ILD nurses to provide support
  • Act as a liaison to the PNC committee, sleep center, and associates involved in sponsored clinical trials
  • Follow all protocols for documentation and maintenance of databases related to the PLN Program
  • Facilitate testing for oxygen and order or perform 6-minute walk tests
  • Assist and delegate to internal staff all general duties
  • Facilitate patient instruction on medication and delivery devices
  • Disclose results of sleep testing to patients and coordinate further care
  • Assess patient's CPAP mask fit and make changes as needed
  • Facilitate in-person CPAP teaching for patients
  • Facilitate all Durable Medical Equipment (DME) orders
  • Maintain a list of insurance companies accepted by each DME provider
  • Facilitate changes in home DME settings
  • Liaison between medical equipment companies and physicians
  • Develop and implement educational materials for patients and families
  • Responsible for overseeing internal staff education and competencies
  • Responsible for overseeing internal staff competencies regarding sleep compliance downloads
  • Maintain currency with all required compliance training programs
  • Assist in the coordination of a post-discharge clinic
  • Coordinate specialty medications initiation paperwork
  • Delegate to ensure daily monitoring of medication refrigeration, emergency box, sample closet, and logbooks
  • Manage all incoming and outgoing surgical clearance requests
  • Identify potential topics for research related to the program
  • Keep up to date on all current literature and studies related to the PLN Program
  • Provide written reports as requested to support research projects
  • Maintain a database of clinical protocols for reference and review
  • Follow all protocols for documentation and maintenance of databases related to the PLN Program
  • Participate in and coordinate outreach to current and potential referring physicians
  • Maintain currency with all required compliance training programs
  • Maintain a commitment to professional growth and competency
Requirements
  • Minimum of 2 years clinical RN experience with patient care required; Pulmonary, Sleep or Critical Care experience preferred
  • Licensed as a Registered Nurse (RN) in the State of Florida
  • Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association
  • Communication and Problem-Solving skills and ability to maintain flexibility
  • Ability to manage, read, and direct patient care and administrative functions in pulmonary and crucial care services
  • Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows
