Case Manager III

Telecare Corporation is seeking a skilled Case Manager III to join our team. As a primary support and case manager, you will work with individuals with mental illness and other co-occurring disorders to help them live independently in their chosen community.
Key Responsibilities- Provide direct supervision and support to individuals with mental illness and other co-occurring disorders
- Assist individuals in developing and implementing individualized treatment plans
- Coordinate with healthcare providers, family members, and other stakeholders to ensure comprehensive care
- Monitor and evaluate the effectiveness of treatment plans and make adjustments as needed
- Document all interactions and progress in client records
- Bachelor's degree in Social Services or Human Services and (1) year of experience or High School Graduation or G.E.D. equivalent and four (4) years of direct services in Mental Health or community services
- Valid and current driver's license, and personal vehicle insurance with your name listed as a driver
- Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
- Willingness to use the company vehicle to drive clients to appointments and groups, etc.
- Willingness to use your personal vehicle to attend meetings, etc.
- Paid Time Off: For Full Time Employee it is 16.7 days in your first year
- Nine Paid Holidays & Shift differentials for hourly staff (6% for PM Shift, 10% for Overnight Shift). Weekend Shift differentials for hourly staff (5% for Weekend AM Shift, 11% for Weekend PM Shift, 15% for Weekend Overnight Shift)
