Key Responsibilities
- Provide high-quality patient care in the home setting, following established protocols and procedures.
- Collaborate with multidisciplinary teams to develop and implement comprehensive care plans.
- Conduct thorough assessments, identify patient needs, and coordinate care delivery.
- Communicate effectively with patients, families, and caregivers to ensure optimal outcomes.
- Stay up-to-date with industry developments, best practices, and regulatory requirements.

Requirements
- Current licensure as a Registered Nurse (RN) in the state of New York.
- 1+ year of experience in Home Health care or related field.
- Strong clinical skills and ability to work independently.
- Excellent communication and interpersonal skills.
